The Preparticipation Physical Packet and ALL Sport Participation Forms must be fully completed and turned in to our Athletic Trainer for approval and clearance by our School Physician before being able to participate in any TCPA-offered sport.

Preparticipation Physicals are good for one (1) year (365 days). A Sports Participation Packet must be completed and handed in every sports season. The packet is posted in the links below.
